You must listen to binaural sound with headphones... for the effect to work, and when it does it is AMAZING!
This video shows us what the process of recording binaural sound looks like. The "Head" is made from 2 pieces of foam glued together. Then an omni directional microphone is placed where the left ear would be and then one where the right ear would be. The result is the bounce and refraction that would normally occur around your head. The entire device is covered in plastic to protect it from the elements. SP-TFB-2 Microphones are recommended by this sound recordist.
In the past i have tried a similar technique by attaching small omni directional microphones on each side of a baseball cap and then wearing the cap so my natural head acts as the deflector. The closer the mics are to your ears the better the audio illusion.

This piece really shows off the potential for strange effects. Imagine how this could be used in feature films etc.

This last one is of a recording session, but its fun to watch and see where the performers are relative to the binaural head. A nice part is when the guy comes in with the shaker, you can really feel his presence.
